aldosteronism?<\/h2>\n\n\n\n<p>PA, an underdiagnosed cause of hypertension, results from excess aldosterone production by the adrenal glands. While crucial for salt balance, elevated levels of aldosterone can lead to excess salt and fluid retention and increased blood pressure. If undiagnosed, people with PA have an increased risk of strokes, heart attacks at a younger age and in some cases heart and kidney failure.<\/p>\n\n\n\n<p>A simple blood test, the aldosterone to renin ration (ARR) is used to screen for this common, potentially curable condition, however it remains underutilised. Despite decades of research into the deleterious effects of untreated PA, global screening for this condition remains low. In collaboration with the Primary Aldosteronism Foundation, this study is using qualitative interviews and surveys to understand current PA knowledge, familiarity with its diagnosis, attitudes towards screening, and perceived barriers and enablers among clinicians who manage hypertension in both primary and tertiary care settings. However, in PA, aldosterone production is unregulated, resulting in inappropriate activation of mineralocorticoid receptors, excess resorption of sodium and water, and hypertension. Diagnosis of PA provides an opportunity for effective targeted treatment with either adrenalectomy or a mineralocorticoid receptor antagonist (spironolactone or eplerenone) to achieve blood pressure control and reduce cardiovascular disease independent of blood pressure. The identification of mutations in adrenal ion channels has opened the door to novel therapies, such as the use of macrolide antibiotics to block mutant potassium channels in PA. However, genetic discoveries remain to be translated into personalised therapies.<\/p>\n<p>In this study, peripheral blood DNA from affected families will be collected and subjected to whole genome sequencing in an effort to elucidate novel gene mutations that cause familial PA. In these two studies, PACE will facilitate collaborations of clinicians, epidemiologists and biostatisticians to translate data from birth cohort studies and large longitudinal datasets to gain an understanding of the natural history of PA.<\/p>\n<ol>\n<li><strong>The Raine Study \u2013 Western Australia, 1989<\/strong><\/li>\n<\/ol>\n<p><strong>Program leads:<\/strong> Professor Trevor Mori, Professor Lawrence Beilin<\/p>\n<p>The Raine Study is one of the largest global birth cohort studies examining the developmental origins of cardiovascular disease. It has already led to the discovery of a novel sex-specific association between the Aldosterone Renin Ratio (ARR) and blood pressure for those aged 17 and 27 years. Recall of the Raine Study cohort, including both the 34-year-old offspring and their parents, will allow repeat ARR measurements and unique insights into the evolution and heritability of PA.<\/p>\n<ol start=\"2\">\n<li><strong>The Aboriginal Birth Cohort \u2013 Northern Territory, 1987<\/strong><\/li>\n<\/ol>\n<p><strong>Program lead:<\/strong> Professor Gurmeet Singh<\/p>\n<p>The Aboriginal Birth Cohort (ABC) includes 686 babies born to Indigenous mothers at the Royal Darwin Hospital with follow-up at four distinct time points. Participants of the ABC and Top End Cohort (TEC; matched, non-Indigenous cohort) will be examined at ages 32-35 years occurring between April 2019 and Dec 2021. This will be the first study to collect data on aldosterone and renin in the Indigenous population with a matched non-Indigenous cohort from the Northern Territory. This study will allow us to gain a greater understanding of the prevalence of aldosterone excess in a First Nations cohort, as well as the relationship between markers of aldosterone excess and cardio-metabolic parameters. At present, there is a growing gap between the demand for testing and lack of capacity for the accurate and timely diagnosis of PA in the tertiary care setting. Early detection of PA, using the current aldosterone to renin ratio (ARR) screening blood test, permits effective treatment or cure. However, the ARR test is significantly underutilised, resulting in hundreds of thousands of Australians with PA missing out on accurate diagnosis and optimal care.<\/p>\n<p>This is a pragmatic, parallel-group, cluster randomised controlled trial that will investigate whether more efficient education strategies, combined with electronic clinical decision support, increases the screening of PA patients at risk. This <a href=\"https:\/\/hudson.org.au\/research-program\/consep\/\"><strong>CONSEP study<\/strong><\/a> will also explore the health and economic implications of routine screening for PA in primary care. If proven effective, subsequent scale-up of this world-first randomised clinical trial will be catalysed by the broadening influence of data platforms in primary care to allow the intervention to be rapidly translated into Australian general practice. our gut may influence blood pressure and how people respond to different treatments. This project aims to explore whether people with primary aldosteronism (PA) have different gut bacteria compared to those with essential (common) hypertension. We will study the gut microbiota of patients with PA before and after they receive targeted treatment, such as adrenal gland surgery or medication that blocks aldosterone. We will also look at whether aldosterone levels are linked to specific patterns of gut bacteria.<\/p>\n<p>Understanding these relationships may reveal new ways that gut bacteria influence blood pressure and treatment response. If it isn&#8217;t diagnosed or treated, this excess hormone can lead to high blood pressure and long-term damage to the heart, kidneys, and other organs.<\/p>\n<p>Our research team is working to understand why this hormone becomes overproduced. In some people, the cause is a small tumour in the adrenal gland. In others, the tendency to develop PA may be inherited, meaning it runs in families.
